Student Self-Portrait
Academic Work Sample
Create a self-portrait. You
may use any medium or art form. Students have submitted essays,
letters, plays, poems, paintings, drawings, sculpture, and collages.
Also, submit a sample of academic work completed for one of
your classes.
Have fun with your self-portrait
and be sure you tell us what you think we should know about
you. Please submit academic work from the 2003-2004 Calendar
year. You may bring both items to the interview, or you may
send it with your Student Questionnaire.

Transcript Release Form
Have your parent sign this
form and send it to your current school's registrar.
Submit to your Registrar
as soon as possible, indicating that a copy of your Official
Transcript be sent to Academe of the Oaks.
Recommendations

Counselor Reference
Complete the top part of
these forms. Give the Counselor form to your school official.
Give the English Teacher and Mathematics Teacher forms (with
envelopes) directly to your teachers.
Be certain teachers know
that we must receive their recommendations as soon as possible,
so that your interview can be scheduled.
Please note,
all the above application materials must be completed and received
by Academe of the Oaks before an interview can be scheduled.

Student Entrance Exam
After receiving all of your
completed application forms, we will call to schedule an interview
with you and one with your parents; schedule a time for the
onsite Entrance Exam.
